Differential Diagnosis for Persistent Polyuria and Urgency
Single Most Likely Diagnosis
- Diabetes Mellitus: This is the most likely diagnosis given the symptoms of polyuria and urgency, which are classic presentations of uncontrolled diabetes. The fact that the urine analysis is clear does not rule out diabetes, as the symptoms can precede the onset of glucosuria.
Other Likely Diagnoses
- Overactive Bladder: This condition is characterized by urgency, frequency, and nocturia, which align with the patient's symptoms. It's a common condition, especially in older adults.
- Interstitial Cystitis: Also known as painful bladder syndrome, this condition can cause urgency, frequency, and pelvic pain, which could be considered in the differential diagnosis, especially if the patient reports pain.
- Hormonal Changes: Postmenopausal women can experience urinary symptoms due to decreased estrogen levels, which can lead to urgency and frequency.
Do Not Miss Diagnoses
- Diabetic Ketoacidosis (DKA): Although less likely given the successful treatment of the UTI and clear urine analysis, DKA can present with polyuria and should be considered, especially if there are any signs of metabolic disturbance.
- Urinary Tract Malignancy: While rare, malignancies such as bladder or renal cancer can cause persistent urinary symptoms and should not be missed, especially in an older adult.
- Neurogenic Bladder: Conditions affecting the nervous system, such as multiple sclerosis or spinal cord injuries, can lead to urinary symptoms. Given the age of the patient, other neurodegenerative diseases could also be considered.
Rare Diagnoses
- Nephrogenic Diabetes Insipidus: A rare condition where the kidneys are unable to concentrate urine, leading to polyuria. It could be considered if other more common causes are ruled out.
- Psychogenic Polydipsia: A condition where excessive fluid intake leads to polyuria. It's less likely but could be a consideration if other diagnoses are excluded and there's a history suggestive of this behavior.
- Granulomatosis with Polyangiitis (formerly known as Wegener's Granulomatosis): A rare autoimmune disorder that can affect the kidneys and cause urinary symptoms among other systemic manifestations.
