Duration of Sexual Abstinence After STI Treatment with Azithromycin or Doxycycline

Abstinence Recommendations by Treatment Regimen
Azithromycin (1g single dose)

- Both medications are highly efficacious for urogenital chlamydial infections, though doxycycline has demonstrated superior efficacy for rectal chlamydia (96.9% vs 76.4% for azithromycin) 2, 3
- For patients treated with erythromycin (an alternative regimen), a test of cure may be considered 3 weeks after completion of treatment 1
Special Considerations
- For pregnant patients, azithromycin 1g as a single dose or amoxicillin 500mg three times daily for 7 days are the recommended regimens 1, 2
- Doxycycline is contraindicated during pregnancy 1, 2
- HIV-positive patients should receive the same treatment regimens as HIV-negative patients 1, 2
Common Pitfalls to Avoid
- Inadequate partner treatment: Failure to ensure all recent partners are treated can lead to reinfection
- Premature resumption of sexual activity: Resuming sexual activity before completing the recommended abstinence period increases risk of transmission
- Assuming cure without adequate time for treatment: Even with single-dose azithromycin, patients should still abstain for the full 7 days
- Overlooking co-infections: Patients with chlamydia often have concurrent gonorrhea requiring dual therapy 2
