Single Most Likely Diagnosis
- Methotrexate-induced neutropenia or bone marrow suppression: Methotrexate is known to suppress the bone marrow, leading to neutropenia, which can increase the risk of infections. Fever can be an early sign of infection in a neutropenic patient, even before other symptoms develop.
Other Likely Diagnoses
- Infection (e.g., urinary tract infection, pneumonia): Although the patient does not have other symptoms, infections can sometimes present with fever alone, especially in the early stages or in immunocompromised patients.
- Viral infection (e.g., influenza, COVID-19): Viral infections can cause fever without other specific symptoms, particularly in the initial phase.

Do Not Miss Diagnoses
