Differential Diagnosis for Fever without Other Symptoms on Methotrexate

Single Most Likely Diagnosis

  • Methotrexate-induced neutropenia or bone marrow suppression: Methotrexate is known to suppress the bone marrow, leading to neutropenia, which can increase the risk of infections. Fever can be an early sign of infection in a neutropenic patient, even before other symptoms develop.

Other Likely Diagnoses

  • Infection (e.g., urinary tract infection, pneumonia): Although the patient does not have other symptoms, infections can sometimes present with fever alone, especially in the early stages or in immunocompromised patients.
  • Viral infection (e.g., influenza, COVID-19): Viral infections can cause fever without other specific symptoms, particularly in the initial phase.
  • Drug fever: Some medications, including methotrexate, can cause drug fever, which is a fever without any identifiable cause other than the medication itself.

Do Not Miss Diagnoses

  • Sepsis: Although sepsis typically presents with additional symptoms such as tachycardia, tachypnea, and hypotension, it can occasionally present with fever alone, especially in the early stages. Missing sepsis can be fatal.
  • Methotrexate toxicity or overdose: High levels of methotrexate can lead to severe toxicity, including bone marrow suppression, liver toxicity, and renal failure. Early detection is crucial.
  • Pneumocystis jirovecii pneumonia (PCP): This opportunistic infection is a concern in immunocompromised patients, including those on methotrexate. It can present with non-specific symptoms, including fever.

Rare Diagnoses

  • Lymphoma: Although rare, patients on long-term immunosuppressive therapy, including methotrexate, have an increased risk of developing lymphomas, which can present with fever among other symptoms.
  • Autoimmune disorders: Certain autoimmune disorders, such as adult-onset Still's disease or systemic lupus erythematosus, can present with fever without other specific symptoms and can be exacerbated or triggered by medications like methotrexate.
