Recommended Dosage of Fexofenadine (Allegra)
For adults and children with allergic rhinitis or chronic idiopathic urticaria, fexofenadine should be dosed at 60 mg twice daily or 180 mg once daily for normal renal function, and reduced to 60 mg once daily in patients with decreased renal function. 1
Adult Dosing
Normal renal function:
- 60 mg twice daily OR
- 180 mg once daily
Impaired renal function:
- 60 mg once daily as the starting dose 1
- No specific creatinine clearance threshold is specified in the FDA label, but dose reduction is recommended for any degree of renal impairment
Pediatric Dosing
Children 12 years and older:

Common Pitfalls to Avoid

Drug interactions: While no clinically significant drug interactions have been identified, be aware that antacids containing aluminum or magnesium may reduce absorption of fexofenadine if taken simultaneously.
