Aripiprazole Titration Schedule
The recommended titration schedule for Abilify (aripiprazole) begins with 2 mg daily for 2 days, then 5 mg for 2 days, followed by the target dose of 10 mg daily, with no further titration needed unless clinically indicated. 1
Adult Dosing for Schizophrenia
Initial Titration
- Starting dose: 10-15 mg once daily
- No titration required for most adults; can start directly at the target dose 1
- However, when side effects are a concern, a more gradual approach may be used:
- Day 1-2: 2 mg daily
- Day 3-4: 5 mg daily
- Day 5 onward: 10 mg daily (target dose)
Maintenance Dosing
- Target dose: 10-15 mg once daily

- With both strong CYP2D6 and CYP3A4 inhibitors: Administer a quarter of usual dose
- With strong CYP3A4 inducers: Double usual dose over 1-2 weeks 1

- Short-term studies show effective doses ranging from 10-20 mg/day
- Long-term studies (16-52 weeks) suggest efficacy at average doses between 15-28 mg/day 3
- High-dose strategies (>15 mg/day) may provide greater symptom reduction, particularly in long-term treatment (>8 weeks) 4
Administration
- Can be taken with or without food
- Once-daily dosing is effective and convenient

- Rapid titration or high initial doses may increase risk of side effects
