How to Take Fosfomycin Sachet

Fosfomycin sachet should be dissolved in 3-4 ounces (1/2 cup) of cold water, stirred until completely dissolved, and consumed immediately. 1

Preparation Instructions

  1. Open the sachet: Take one single-dose sachet of fosfomycin tromethamine granules.
  2. Prepare water: Pour 3-4 ounces (approximately 1/2 cup) of water into a glass.
    • Use only cold or room temperature water
    • Do NOT use hot water as this may affect the medication's efficacy
  3. Mix thoroughly: Pour the entire contents of the sachet into the water and stir until completely dissolved.
  4. Drink immediately: Consume the entire solution right away after mixing.

Important Administration Notes

  • Fosfomycin can be taken with or without food 1
  • The medication should NEVER be consumed in its dry form 1
  • For uncomplicated urinary tract infections in women, only a single 3-gram sachet is typically needed 1, 2
  • No special timing relative to meals is required

What to Expect

  • Fosfomycin is primarily used for treating uncomplicated urinary tract infections (acute cystitis) 3, 2
  • The medication reaches peak concentration in the urine within a few hours and remains at effective levels for up to 48 hours after a single dose 2
  • Most common side effect is mild gastrointestinal distress 4

Special Considerations

  • No dosage adjustments are required for elderly patients, pregnant women, or those with renal or hepatic impairment 2
  • If you have severe kidney disease, inform your healthcare provider before taking this medication
  • For recurrent UTIs, fosfomycin may be prescribed as a prophylactic treatment at 3g every 10 days 3

Fosfomycin's single-dose regimen offers excellent convenience and compliance advantages compared to multi-day antibiotic courses for uncomplicated UTIs, while maintaining comparable efficacy to other first-line treatments 2.
