First-Line Treatment Options
For Patients with Ear Tubes or Perforated Tympanic Membranes:

Important Considerations:
- Topical antibiotic drops are more effective than oral antibiotics for treating ear drainage in patients with tubes 1
- Avoid aminoglycoside-containing drops in patients with perforated tympanic membranes due to potential ototoxicity 1
- Staphylococcus aureus is one of the most common pathogens in ear discharge, along with Pseudomonas aeruginosa 1
Proper Administration Technique
- Warm drops by holding bottle in hand for 1-2 minutes
- Position patient with affected ear upward
- Instill prescribed number of drops to fill ear canal
- Maintain position for 3-5 minutes to facilitate penetration
- Pump the tragus 4 times to help medication penetrate (especially important with tympanic membrane perforation) 1
Precautions During Treatment
- Keep ear dry during treatment
- Use earplugs or cotton with petroleum jelly when showering
- Dry ears thoroughly after swimming or bathing
- Consider acidifying ear drops before/after swimming if necessary 1
Monitoring and Follow-up
- Schedule follow-up in 14 days to assess treatment response
- Consider referral to an otolaryngologist if symptoms persist despite appropriate therapy for 2 weeks 1
- Be vigilant for ciprofloxacin resistance, which has been reported in 4.5% of patients with otorrhea and is increasing 1
