From the Guidelines

It is generally not recommended to stop trazodone abruptly, and instead, it should be tapered off gradually under medical supervision to minimize withdrawal symptoms. Trazodone, a triazolopyridine antidepressant, affects serotonin levels in the brain, and sudden discontinuation may lead to withdrawal symptoms such as anxiety, agitation, sleep disturbances, headache, irritability, and in some cases, flu-like symptoms 1. Although the provided evidence primarily focuses on SSRIs, the principle of gradual tapering to avoid discontinuation syndrome can be applied to other serotonergic medications like trazodone.

Key Considerations for Trazodone Discontinuation

  • The tapering schedule depends on the current dose, how long the medication has been taken, and individual health factors.
  • A common approach might involve reducing the dose by 25-50mg every 3-7 days, but this should be personalized by a healthcare provider.
  • Even if taking a low dose for sleep rather than depression, tapering is still advisable to avoid potential withdrawal symptoms.
  • Medical education, training, and experience are necessary to safely and effectively manage the discontinuation of antidepressant medications, including trazodone 1.

Importance of Medical Supervision

Given the potential for withdrawal symptoms and the importance of adjusting the tapering schedule based on individual response, consulting with a healthcare provider to develop an appropriate discontinuation plan is crucial. This ensures that the discontinuation is managed safely and effectively, minimizing the risk of adverse effects while protecting the patient's quality of life and reducing morbidity and mortality risks associated with improper discontinuation of serotonergic medications.

From the FDA Drug Label

Adverse reactions may occur upon discontinuation of Trazodone Hydrochloride Tablets [See WARNINGS AND PRECAUTIONS (5.8)]. Gradually reduce the dosage rather than stopping Trazodone Hydrochloride Tablets abruptly whenever possible. Advise patients not to abruptly discontinue Trazodone Hydrochloride Tablets and to discuss any tapering regimen with their healthcare provider. Adverse reactions can occur when Trazodone Hydrochloride Tablets are discontinued [see Warnings and Precautions (5. 8)].

Trazodone should not be discontinued abruptly. The dosage should be gradually reduced whenever possible to minimize the risk of adverse reactions upon discontinuation 2 2.

From the Research

Discontinuation of Trazodone

  • Trazodone is a triazolopyridine antidepressant that may require gradual tapering to minimize discontinuation symptoms 3.
  • Abrupt discontinuation of trazodone can lead to withdrawal symptoms, including noradrenergic rebound, due to its effects on the serotonergic system and short half-life of the drug and its metabolite m-chlorophenylpiperazine 3.
  • Gradual tapering of trazodone at a very slow rate is recommended to avoid withdrawal symptoms 3.

General Guidance on Discontinuing Antidepressants

  • Gradual tapering is necessary for all serotonin reuptake inhibitors (SRIs) except fluoxetine to minimize discontinuation symptoms 4.
  • Discontinuation symptoms can be mild, short-lived, and self-limiting, but may also be distressing and lead to decreased productivity 4.
  • Healthcare professionals should be educated about the potential adverse effects of SRI discontinuation and provide guidance on gradual tapering 4, 5, 6.
  • Patients should be informed about potential withdrawal symptoms and regularly reviewed during the tapering process 5, 6.

Importance of Gradual Tapering

  • Abrupt discontinuation of psychotropic drugs, including antidepressants, can lead to rebound effects or withdrawal symptoms 6, 7.
  • Gradual tapering is necessary to minimize adverse discontinuation effects, including mood instability and manic relapse in patients with bipolar disorder 7.
  • Patients should be educated about the possible adverse effects of abrupt medication discontinuation and the importance of gradual tapering 7.
