Recommended Aftercare for Wasp Sting Swelling
For wasp sting swelling, immediate application of cold compresses, elevation of the affected area, and administration of oral antihistamines are the recommended first-line treatments to reduce pain and swelling.
Immediate Management
- Remove the stinger if still present (scrape it out with a credit card or fingernail, don't use tweezers which can squeeze more venom into the skin)
- Clean the area with mild soap and water to prevent secondary infection
- Apply cold compresses for 20 minutes at a time to reduce swelling
- Elevate the affected limb if possible to minimize swelling
Medication Management
- Oral antihistamines (such as diphenhydramine/Benadryl) to reduce itching and swelling
- Over-the-counter pain relievers like acetaminophen or ibuprofen for pain management
- Topical hydrocortisone cream (0.5-1%) can help reduce local inflammation
- Avoid scratching the area to prevent secondary infection
When to Seek Medical Attention
Seek immediate medical attention if any of the following occur:
- Signs of anaphylaxis (difficulty breathing, throat swelling, dizziness, rapid heartbeat)
- Progressive swelling that extends beyond the sting site
- Development of neurological symptoms (headache, disorientation, seizures) which may indicate rare complications such as allergic encephalitis 1
- Multiple stings, which can lead to more severe systemic reactions including vomiting, diarrhea, generalized edema, or hypotension 2
- Delayed allergic reactions, which can occur 1-2 weeks after the sting 3
Special Considerations
- Patients with a history of severe allergic reactions to insect stings should carry an epinephrine auto-injector
- Some patients may develop unusual complications such as eosinophilic cellulitis (Wells syndrome) that doesn't respond to antibiotics but requires systemic corticosteroids 4
- For stings in sensitive areas like the eye, topical corticosteroids may be necessary under medical supervision 5
Prevention of Future Stings
- Wear protective clothing (long pants, long sleeves, closed shoes) when in areas where wasps may be present
- Use insect repellents containing DEET
- Apply permethrin spray on clothing for additional protection
- Avoid wearing bright colors or floral patterns that may attract wasps
- Be cautious with food and sweet drinks outdoors, which can attract wasps
