Tetanus Immunoglobulin (TIG) Administration Guidelines
Tetanus Immunoglobulin (TIG) should be administered if your tetanus vaccination is not up to date AND you have a wound that is contaminated, deep, or at high risk for tetanus infection. 1, 2
Decision Algorithm for TIG Administration
The need for TIG depends on both your vaccination history and the type of wound you have:
Based on Vaccination History:
Unknown or fewer than 3 doses of tetanus vaccine:
- Clean, minor wound: Tetanus toxoid-containing vaccine only (no TIG)
- All other wounds: BOTH tetanus toxoid-containing vaccine AND TIG 1
3 or more doses of tetanus vaccine:
Special Considerations:
- Immunocompromised patients: Should receive TIG for contaminated wounds regardless of vaccination history 2
- Pregnant women: Should receive Tdap if tetanus prophylaxis is indicated 2
Types of Wounds Requiring TIG Consideration
TIG should be considered for "all other wounds" which include:
- Wounds contaminated with dirt, feces, soil, or saliva
- Puncture wounds
- Avulsions
- Wounds resulting from missiles, crushing, burns, or frostbite 1, 2, 3
Administration Details
When TIG is indicated:
- Standard prophylactic dose: 250 units administered intramuscularly as a single dose 2, 3
- When both TIG and tetanus toxoid-containing vaccine are needed, they should be administered using separate syringes at different anatomical sites 1, 2
Important Caveats
- Allergic reactions: TIG is contraindicated if you have a severe allergic reaction to a previous dose or vaccine component 2
- Timing considerations: TIG provides passive immunity that lasts approximately 4 weeks 4
- Interference with vaccine response: TIG may slightly reduce the immune response to simultaneously administered tetanus vaccine, but most patients still achieve protective antibody levels 5
- Proper wound care: Immediate and thorough wound cleaning is critical for tetanus prevention regardless of TIG administration 2
Common Pitfalls to Avoid
- Unnecessary administration: TIG is not needed for all wounds or for fully vaccinated individuals with recent boosters 6
- Delayed administration: If indicated, TIG should be given promptly as it provides immediate passive protection 3
- Relying solely on TIG: TIG provides only temporary protection; completing the tetanus vaccination series is essential for long-term immunity 3, 7
