Recommended Dilution Rate for Monocef (Ceftriaxone)

For intravenous administration of Monocef (Ceftriaxone), reconstitute with appropriate diluent to achieve a concentration between 10 mg/mL and 40 mg/mL, with 100 mg/mL being the standard concentration after initial reconstitution. 1

Reconstitution Guidelines

Initial Reconstitution

  • For IV administration, reconstitute vials with appropriate IV diluent according to the following table 1:

    • 250 mg vial: add 2.4 mL diluent
    • 500 mg vial: add 4.8 mL diluent
    • 1 gram vial: add 9.6 mL diluent
    • 2 gram vial: add 19.2 mL diluent
  • After initial reconstitution, each 1 mL of solution contains approximately 100 mg of ceftriaxone 1

Administration Method

  • Administer intravenously by infusion over 30 minutes in adults and children 1
  • For neonates, administer over 60 minutes to reduce the risk of bilirubin encephalopathy 1

Compatible Diluents

  • Appropriate IV diluents include 1:
    • Sterile Water for Injection
    • 0.9% Sodium Chloride Solution
    • 5% Dextrose Solution

Important Compatibility Warnings

  • Do not use diluents containing calcium (such as Ringer's solution or Hartmann's solution) to reconstitute ceftriaxone or to further dilute for IV administration as particulate formation can result 1

  • Ceftriaxone is incompatible with the following medications in admixtures 1:

    • Vancomycin
    • Amsacrine
    • Aminoglycosides
    • Fluconazole
  • When any of these drugs need to be administered with ceftriaxone, administer them sequentially with thorough flushing of IV lines between administrations 1

Stability After Reconstitution

  • Reconstituted ceftriaxone solutions remain stable (less than 10% loss of potency) for the following time periods 1:
    • At 100 mg/mL concentration:
      • Room temperature (25°C): 2 days
      • Refrigerated (4°C): 10 days
    • At 250-350 mg/mL concentration:
      • Room temperature (25°C): 24 hours
      • Refrigerated (4°C): 3 days

Special Considerations

  • The color of reconstituted solutions ranges from light yellow to amber, depending on storage time, concentration, and diluent used 1
  • After reconstitution, protection from light is not necessary 1
  • Ceftriaxone has been shown to be compatible with metronidazole hydrochloride at concentrations not exceeding 5-7.5 mg/mL metronidazole with ceftriaxone 10 mg/mL 1

Following these dilution guidelines will ensure proper administration of Monocef (Ceftriaxone) while minimizing the risk of adverse reactions or medication incompatibilities.
