Vagal Maneuver Techniques and Duration
Vagal maneuvers are the first-line intervention for terminating supraventricular tachycardia (SVT) and should be performed with the patient in the supine position. The two main types of vagal maneuvers used clinically are:
Carotid Sinus Massage
- Duration: 5 to 10 seconds 1
- Technique: Apply steady pressure over the right or left carotid sinus after confirming absence of carotid bruit by auscultation 1
- Contraindications: Patients with diseased carotid arteries due to risk of cerebrovascular accident 2
Valsalva Maneuver
- Duration: 10 to 30 seconds 1
- Technique: Patient raises intrathoracic pressure by bearing down against a closed glottis, equivalent to at least 30-40 mmHg 1
- Effectiveness: Studies show Valsalva is generally more successful than carotid sinus massage 1
Clinical Considerations
- Vagal maneuvers should be performed before attempting pharmacological interventions for SVT 1
- These maneuvers will typically not be effective if the rhythm does not involve the AV node as a requisite component of a reentrant circuit 1
- Switching between techniques (Valsalva to carotid massage or vice versa) can increase overall success rate to approximately 27.7% 1
- Alternative vagal maneuver: Applying an ice-cold, wet towel to the face (based on the diving reflex) 1
Special Populations
Pregnant Patients
- The same durations apply for pregnant patients: 5-10 seconds for carotid sinus massage and 10-30 seconds for Valsalva maneuver 1
- Vagal maneuvers remain the first-line intervention for SVT in pregnancy 1
Important Cautions
- The practice of applying pressure to the eyeball is potentially dangerous and has been abandoned 1
- In rare instances, carotid sinus massage may initiate ventricular tachycardia 2
- Always ensure availability of emergency equipment when performing vagal maneuvers, particularly in patients with known cardiac conditions 1
