How to Inject Trulicity (Dulaglutide)
To properly administer Trulicity, inject it subcutaneously in the abdomen, thigh, or upper arm, rotating injection sites with each weekly dose. 1
Preparation Steps
- Inspect Trulicity visually before use - it should appear clear and colorless. Do not use if particulate matter or coloration is seen 1
- Allow refrigerated Trulicity to warm up to room temperature before injection for more comfortable administration 2
- Clean the injection site with an alcohol swab before injection 2
- No need to resuspend or shake Trulicity as it is a clear solution (unlike cloudy insulins that require mixing) 2
Injection Technique
- Use the single-dose pen provided with Trulicity 1
- Inject subcutaneously at a 90° angle into the abdomen, thigh, or upper arm 1
- For thin individuals, consider pinching a skinfold and injecting at a 45° angle to avoid intramuscular injection 2
- When creating a skinfold, lift the skin gently with thumb and index finger without squeezing too tightly to avoid blanching or pain 2
- Hold the needle in place for a count of 10 after the plunger is depressed to ensure complete delivery 2
- Withdraw the needle at the same angle it was inserted 2
- Apply mild pressure to the injection site for about 1 minute after removing the needle to prevent leakage 2
Important Administration Guidelines
- Administer Trulicity once weekly, any time of day, with or without food 1
- The recommended starting dose is 0.75 mg injected subcutaneously once weekly 1
- For adults, the dose may be increased to 1.5 mg once weekly for additional glycemic control, with further increases up to 4.5 mg possible 1
- For pediatric patients (10 years and older), the maximum recommended dose is 1.5 mg once weekly 1
- Rotate injection sites with each dose to prevent tissue damage 1
- Absorption is most rapid from the abdomen, slower from the upper arm, and slowest from the thigh 3
Managing Missed Doses
- If a dose is missed, administer it as soon as possible if there are at least 3 days (72 hours) until the next scheduled dose 1
- If less than 3 days remain before the next scheduled dose, skip the missed dose and administer the next dose on the regularly scheduled day 1
- The day of weekly administration can be changed if necessary, as long as the last dose was administered 3 or more days before the new day of administration 1
Special Considerations
- When using Trulicity with insulin, administer as separate injections and never mix them 1
- While it's acceptable to inject Trulicity and insulin in the same body region, the injections should not be adjacent to each other 1
- Store unopened Trulicity in a refrigerator; after initial use, it can be stored at room temperature (15-30°C or 59-86°F) according to manufacturer's recommendations 2
- Avoid exposing Trulicity to direct heat, light, or excessive agitation 2
Common Pitfalls to Avoid
- Avoid intramuscular injection as it can lead to unpredictable absorption and variable effects on glucose 2
- Do not reuse needles as this increases risk of infection and tissue damage 2
- Avoid injecting into areas of lipohypertrophy (accumulated fat under the skin from repeated injections in the same spot) 2
- Never shake Trulicity vigorously as this can create bubbles that affect accurate dosing 2
