Recommended Strength for Ear Drops
All FDA-approved ear drops for treating acute otitis externa (AOE) are highly effective, with no consistent advantage shown for any one specific drug. 1
Standard Dosing for Common Ear Drops
Ciprofloxacin Otic Solution 0.2%
- Instill contents of one single-dose container (0.25 mL) into the affected ear twice daily for 7 days 2
- Warm solution by holding container in hands for at least 1 minute before instillation to minimize dizziness 2
Ofloxacin Otic Solution 0.3%
- For children (6 months to 13 years): Five drops (0.25 mL) instilled into the affected ear once daily for 7 days 3, 4
- For patients 13 years and older: Ten drops (0.5 mL) instilled into the affected ear once daily for 7 days 3, 4
- Warm solution before instillation to avoid dizziness 3
Proper Administration Technique
- Have someone else administer the drops when possible (improves adherence to therapy) 1
- Lie down with affected ear upward 1
- Fill ear canal completely with prescribed number of drops 1
- Remain in position for 3-5 minutes to ensure adequate penetration 1
- Use gentle to-and-fro movement of the ear or press with an in/out movement on the tragus to help drops reach intended destination 1
Duration of Treatment
- Use ear drops for at least 7 days, even if symptoms improve sooner, to prevent relapse 1
- If symptoms persist beyond 7 days, notify your doctor and continue drops until symptoms resolve (maximum 7 additional days) 1
Special Considerations
Obstructed Ear Canal
- If ear canal is obstructed by debris or edema, aural toilet (cleaning) or wick placement may be necessary to enhance drug delivery 1
- A wick helps deliver medication through the length of the ear canal when significant edema is present 1
- Do not remove a wick unless instructed to do so by your healthcare provider 1
Perforated Tympanic Membrane
- Inform your doctor if you have had prior ear surgery, have a perforated eardrum, or have ear tubes in place 1
- In these cases, only ear drops approved for middle ear use should be prescribed 1
Alternative and Complementary Treatments
- Home remedies like isopropyl alcohol or 5% acetic acid (white vinegar) mixed with equal parts of alcohol or water have not been formally evaluated in clinical trials 1
- Ear candles should never be used as they have no proven efficacy and can cause harm 1
Common Side Effects
- Ear drops are generally safe and well-tolerated 1
- Most common side effects include local rash, itching, irritation, or discomfort (reported in approximately 2-3% of patients) 2
