Recommended Dosage for Foracit (Ciprofloxacin or Ofloxacin)

For ofloxacin, the standard adult dose is 200-400 mg orally every 12 hours, with specific dosing based on the type of infection being treated. 1

Ofloxacin Dosing

Adult Dosing

  • For uncomplicated urinary tract infections (UTIs):
    • 200 mg orally every 12 hours for 3 days (E. coli or K. pneumoniae) or 7 days (other pathogens) 1
  • For complicated UTIs:
    • 200 mg orally every 12 hours for 10 days 1
  • For acute, uncomplicated gonorrhea:
    • 400 mg as a single oral dose 2, 1
  • For nongonococcal cervicitis/urethritis due to C. trachomatis:
    • 300 mg orally every 12 hours for 7 days 1
  • For pelvic inflammatory disease:
    • 400 mg orally every 12 hours for 10-14 days 1
  • For prostatitis:
    • 300 mg orally every 12 hours for 6 weeks 1

Dosing in Special Populations

  • For patients with renal impairment:
    • Creatinine clearance 20-50 mL/min: usual dose every 24 hours 1
    • Creatinine clearance <20 mL/min: half the usual dose every 24 hours 1
  • For patients with severe liver disorders:
    • Maximum dose should not exceed 400 mg per day 1

Ciprofloxacin Dosing

Adult Dosing

  • For urinary tract infections:
    • 250-500 mg orally every 12 hours 2
  • For uncomplicated gonorrhea:
    • 500 mg orally as a single dose 2
  • For systemic infections:
    • 500 mg orally every 12 hours 2
  • For intravenous administration:
    • 400 mg IV every 12 hours 2

Pediatric Dosing (when necessary)

  • Oral administration:
    • 10-20 mg/kg/dose PO every 12 hours (maximum 750 mg/dose) 2
  • Intravenous administration:
    • 10 mg/kg/dose IV every 8 or 12 hours (maximum 400 mg/dose) 2

Important Considerations

  • Fluoroquinolones should be used judiciously to prevent development of resistance 2
  • Antacids containing calcium, magnesium, or aluminum; sucralfate; divalent or trivalent cations; or multivitamins containing zinc should not be taken within 2 hours before or after taking ofloxacin 1
  • Ofloxacin has almost 100% bioavailability with peak serum concentrations obtained 1-2 hours following oral dosing 3
  • Both ciprofloxacin and ofloxacin are generally well-tolerated, with most common adverse effects being gastrointestinal complaints, headache, and dizziness 4, 5
  • For topical applications (such as otitis externa), ofloxacin otic solution 0.3% can be administered once daily for 7 days 6

Clinical Pearls

  • When treating gonorrhea, if chlamydial infection is not ruled out, add azithromycin 1g orally in a single dose or doxycycline 100 mg orally twice daily for 7 days 2
  • For disseminated gonococcal infection, initial parenteral therapy is recommended with ceftriaxone, but ofloxacin 400 mg IV every 12 hours can be used as an alternative regimen 2
  • Fluoroquinolones are contraindicated in children, pregnant women, and nursing mothers due to potential effects on developing cartilage 3
  • Resistance patterns should be considered when selecting fluoroquinolones for empiric therapy 2
