Management of Liver Cysts
Follow-up imaging is not recommended for asymptomatic simple hepatic cysts regardless of size. 1
Types of Liver Cysts and Their Management
Simple hepatic cysts are benign lesions that typically follow an indolent course without significant changes in size over time. The management approach depends on whether the cyst is:

Post-Treatment Considerations
- Routine follow-up imaging after aspiration sclerotherapy or surgical procedures is not recommended 1, 3
- Treatment success is defined by symptom relief rather than volume reduction of hepatic cysts 1
- If imaging is performed post-treatment, CT or MRI allows for estimation of remnant cyst volume 1
Common Pitfalls to Avoid
- Unnecessary follow-up imaging for asymptomatic simple cysts, which can lead to patient anxiety and healthcare resource waste 1, 2
- Failure to differentiate simple cysts from complex cysts that may require further evaluation 4
- Cysts with irregular walls, septations, calcifications, or daughter cysts should be evaluated with enhanced CT or MRI to rule out cystic neoplasms or hydatid cysts 2
Remember that simple hepatic cysts are common incidental findings (prevalence up to 15-18% in the US) 4, and the vast majority require no intervention or follow-up.
