Sumatriptan Starting Dose for Migraine Treatment
The recommended starting dose of sumatriptan tablets for migraine treatment is 25 mg. 1
Dosing Options by Route of Administration
- For oral administration, the FDA recommends 25 mg, 50 mg, or 100 mg as starting doses, with 25 mg being the lowest effective dose 1
- For subcutaneous administration, the recommended dose is 6 mg, which can be repeated after one hour if needed, with a maximum daily dose of 12 mg per 24 hours 2
- For intranasal administration, the recommended dose is 5-10 mg (one to two sprays) in one nostril, which can be repeated after 2 hours, with a maximum daily dose of 40 mg per day 2
Dose Selection Considerations
- While 25 mg is the starting dose, doses of 50 mg and 100 mg may provide greater effect, though 100 mg may not provide additional benefit over 50 mg 1
- Higher doses carry a greater risk of adverse reactions 1
- In a patient preference study, 35% of patients preferred the 100 mg dose, 31% preferred the 50 mg dose, and 25% preferred the 25 mg dose 3
- Efficacy and speed of action were the main reasons patients preferred higher doses 3
Dosing Schedule
- If migraine has not resolved within 2 hours after taking sumatriptan tablets, or returns after temporary improvement, a second dose may be administered at least 2 hours after the first dose 1
- The maximum daily dose is 200 mg in a 24-hour period 1
- Approximately 40% of patients who initially respond to oral sumatriptan experience recurrence of headache within 24 hours, which can be effectively treated with a further dose 4
Special Populations
- For patients with mild to moderate hepatic impairment, the maximum single dose should not exceed 50 mg 1
- Sumatriptan is contraindicated in patients with ischemic heart disease, previous myocardial infarction, Prinzmetal (variant) angina, and uncontrolled hypertension 2
- Sumatriptan should not be used with ergotamine, MAOIs, or within 24 hours of another triptan 2
- Sumatriptan is also contraindicated in patients with hemiplegic or basilar migraine and during pregnancy 2
Efficacy and Tolerability
- Clinical trials show that sumatriptan 25 mg provides significant headache relief at 2 hours compared to placebo (Number-Needed-to-Treat: 3.4) 5
- All doses (25 mg, 50 mg, and 100 mg) were effective and generally well tolerated in clinical trials 6
- Adverse events are more common with sumatriptan 100 mg than with placebo (Number-Needed-to-Harm: 7.1) 5
- Common adverse events include nausea, vomiting, malaise, fatigue, and dizziness 4
- Chest symptoms (tightness and pressure) occur in 3-5% of patients 4
