How to Administer an 80mg IV Bolus of Protonix (Pantoprazole)
For an 80mg IV bolus of pantoprazole (Protonix), reconstitute with 10mL of 0.9% Sodium Chloride Injection per vial (two 40mg vials) to a concentration of 4mg/mL, and administer intravenously over at least 2 minutes.
Preparation Instructions for 80mg IV Bolus
- Use two 40mg vials of Pantoprazole Sodium for Injection 1
- For each vial, reconstitute with 10mL of 0.9% Sodium Chloride Injection, USP, to achieve a concentration of approximately 4mg/mL 1
- Visually inspect the reconstituted solution for particulate matter and discoloration prior to and during administration 1
- The reconstituted solution may be stored for up to 24 hours at room temperature prior to administration and does not need to be protected from light 1
Administration Technique
- Administer the total volume from both vials (20mL total) intravenously over a period of at least 2 minutes 1
- Administer through a dedicated line or through a Y-site 1
- Flush the intravenous line before and after administration with either 5% Dextrose Injection, USP, 0.9% Sodium Chloride Injection, USP, or Lactated Ringer's Injection, USP 1
Important Compatibility Considerations
- Pantoprazole is compatible with the following solutions when administered through a Y-site: 5% Dextrose Injection, USP, 0.9% Sodium Chloride Injection, USP, or Lactated Ringer's Injection, USP 1
- Midazolam HCl has been shown to be incompatible with Y-site administration of Pantoprazole Sodium for Injection 1
- Pantoprazole may not be compatible with products containing zinc 1
- If administered through a Y-site, immediately discontinue the infusion if precipitation or discoloration occurs 1
Clinical Considerations
- The 80mg IV bolus dose is typically used for pathological hypersecretory conditions including Zollinger-Ellison Syndrome 1, 2
- For standard GERD treatment, the typical dose is 40mg IV, not 80mg 1
- Monitor for potential adverse effects, though pantoprazole is generally well tolerated in both short and long-term administration 3
- Pantoprazole shows linear pharmacokinetics after both IV and oral administration with a serum elimination half-life of about 1.1 hours 4
Storage Considerations
- Do not freeze either the reconstituted or diluted solutions 1
