Differential Diagnosis
Based on the patient's complex medical history and recent normal diagnostic tests, the following differential diagnoses are considered:
- Single most likely diagnosis
- Chronic Intestinal Pseudo-Obstruction (CIPO): Given the patient's history of multiple abdominal surgeries, including LADD with DDJ, and normal recent imaging studies, CIPO is a possible diagnosis. CIPO is a condition characterized by recurrent episodes of intestinal obstruction without any mechanical obstruction, which could be related to the patient's surgical history and underlying conditions such as EDS.
- Other Likely diagnoses
- Gastroparesis: The patient's history of multiple surgeries, including LADD with DDJ, and normal imaging studies, makes gastroparesis a possible diagnosis. Gastroparesis is a condition characterized by delayed gastric emptying, which could be related to the patient's surgical history and underlying conditions.
- Small Intestine Bacterial Overgrowth (SIBO): SIBO is a possible diagnosis given the patient's history of abdominal surgeries and normal imaging studies. SIBO is a condition characterized by an overgrowth of bacteria in the small intestine, which could be related to the patient's surgical history and underlying conditions.
- Ehlers-Danlos Syndrome (EDS) related gastrointestinal symptoms: The patient's history of EDS and multiple surgeries makes it possible that her gastrointestinal symptoms are related to her underlying EDS.
- Do Not Miss (ddxs that may not be likely, but would be deadly if missed.)
- Mesenteric Ischemia: Although the patient has had normal recent imaging studies, mesenteric ischemia is a possible diagnosis that should not be missed. Mesenteric ischemia is a condition characterized by reduced blood flow to the intestines, which could be related to the patient's history of vascular compressions and surgeries.
- Intestinal Lymphangiectasia: Intestinal lymphangiectasia is a rare condition characterized by dilated lymphatic vessels in the intestines, which could be related to the patient's history of EDS and multiple surgeries.
- Rare diagnoses
- Mitochondrial Neurogastrointestinal Encephalomyopathy (MNGIE): MNGIE is a rare genetic disorder characterized by gastrointestinal dysmotility, which could be related to the patient's history of EDS and multiple surgeries.
- Scleroderma: Scleroderma is a rare autoimmune disorder that can cause gastrointestinal symptoms, which could be related to the patient's history of EDS and multiple surgeries.
