Recommended Dosage and Usage of Imitrex (Sumatriptan) for Migraine Headaches
For acute migraine attacks, oral sumatriptan is recommended at doses of 25 mg, 50 mg, or 100 mg, with a maximum daily dose of 200 mg in a 24-hour period. 1
Oral Administration
- The recommended oral doses are 25 mg, 50 mg, or 100 mg, with 50 mg being an effective starting dose for most patients 1
- If the migraine has not resolved after 2 hours or returns after temporary improvement, a second dose may be administered at least 2 hours after the first dose 1
- The 50 mg dose provides clinically useful pain relief in approximately 28% of patients compared to 11% with placebo 2
- The 100 mg dose may provide greater efficacy than 50 mg, but with potentially more adverse effects 1
- For patients with mild to moderate hepatic impairment, the maximum single dose should not exceed 50 mg 1
Alternative Routes of Administration
- Subcutaneous: 6 mg dose (can be repeated after one hour, maximum 12 mg per 24 hours) 3
- Intranasal: 5-10 mg dose (can be repeated after 2 hours, maximum 40 mg per day) 3
- Subcutaneous administration provides more rapid pain relief than other routes but has more adverse effects 2
- Consider using a non-oral triptan and an antiemetic in patients experiencing severe nausea or vomiting 4
Treatment Strategy
- Begin treatment as soon as possible after migraine onset for improved efficacy 4
- Consider combination therapy with an NSAID or acetaminophen for better efficacy 4
- Patients should try sumatriptan for 2-3 headache episodes before abandoning this therapy 3, 5
- If one triptan is ineffective or poorly tolerated, another triptan may be effective 4
Important Contraindications
- Do not use sumatriptan with ergotamine, MAOIs, or within 24 hours of another triptan 3
- Avoid use in patients with hemiplegic or basilar migraine, pregnancy, impaired hepatic function, or coronary artery disease 3
- Do not use opioids or butalbital for migraine treatment 4
Common Adverse Effects
- Adverse events are generally mild to moderate and of short duration 2
- More common with higher doses and subcutaneous administration 2
- Common side effects include nausea, vomiting, malaise, fatigue, dizziness, and chest pressure/heaviness 5, 6
- Subcutaneous administration can cause injection site reactions in approximately 30% of patients 3
Special Considerations
- Be aware of medication overuse headache risk (≥10 days per month with triptans) 4
- Taking medication early, when pain is mild, is more effective than waiting until pain is moderate or severe 2
- If episodic migraines occur frequently or treatment response is inadequate, consider adding preventive medications 4
- The safety of treating more than 4 headaches in a 30-day period has not been established 1
