DOSAGE AND ADMINISTRATION Dosage should be individualized for maximum beneficial effect. Treatment for patients with anxiety should be initiated with a dose of 0.25 to 0.5 mg given three times daily. The dose may be increased to achieve a maximum therapeutic effect, at intervals of 3 to 4 days, to a maximum daily dose of 4 mg, given in divided doses. The successful treatment of many panic disorder patients has required the use of alprazolam tablets at doses greater than 4 mg daily. In controlled trials conducted to establish the efficacy of alprazolam tablets in panic disorder, doses in the range of 1 to 10 mg daily were used Treatment may be initiated with a dose of 0.5 mg three times daily. Depending on the response, the dose may be increased at intervals of 3 to 4 days in increments of no more than 1 mg per day.

The recommended dose and frequency of Alprazolam (Xanax) is:

  • Initial dose: 0.25 to 0.5 mg given three times daily for anxiety, and 0.5 mg three times daily for panic disorder
  • Maximum daily dose: 4 mg given in divided doses for anxiety, and up to 10 mg daily for panic disorder
  • Dose titration: Increase dose at intervals of 3 to 4 days in increments of no more than 1 mg per day
  • Dose maintenance: Periodic reassessment and consideration of dosage reduction is advised for patients receiving doses greater than 4 mg/day 1

From the Research

The recommended dose and frequency of Alprazolam (Xanax) should be individualized and typically starts with 0.25 to 0.5 mg taken three times daily, as this dosage range is commonly cited in the literature as a starting point for adults with anxiety 2, 3. When considering the dosage of Alprazolam, it's crucial to take into account the patient's condition, age, weight, other medications, and medical history. The medication should be taken exactly as prescribed, never more frequently or in higher doses. Some key points to consider when prescribing Alprazolam include:

  • The medication works by enhancing the effects of GABA, a neurotransmitter that inhibits brain activity, helping reduce anxiety and panic.
  • Alprazolam can be habit-forming and may cause side effects like drowsiness or dizziness.
  • The medication should never be stopped abruptly, as withdrawal symptoms can be severe.
  • The optimal therapeutic concentration range for Alprazolam is not clearly established, but some studies indicate that optimal reduction of anxiety associated with panic disorder occurs at steady-state plasma Alprazolam concentrations of 20 to 40 micrograms/L 2. It's essential to consult with a healthcare provider for personalized dosing instructions and monitoring, as they can assess the individual's specific needs and adjust the dosage accordingly. Additionally, it's worth noting that the pharmacokinetics of Alprazolam are not significantly influenced by gender, but clearance is reduced in many elderly individuals and in patients with cirrhosis 2, 3. Overall, the goal of Alprazolam treatment is to improve the patient's quality of life by reducing anxiety and panic symptoms while minimizing the risk of adverse effects and dependence.
