Slynd (Drospirenone) Dosing and Administration

The recommended dose of Slynd (drospirenone) is 4 mg taken orally once daily in a 24/4 regimen, which means 24 days of active tablets followed by 4 days of inactive tablets. 1, 2

Dosing Schedule

  • Take one 4 mg tablet daily for 24 consecutive days, followed by 4 inactive tablets 2
  • The 24/4 regimen was specifically designed to improve bleeding profile and maintain efficacy even with missed doses 2
  • Drospirenone has a half-life of 30-34 hours, which helps maintain contraceptive efficacy during the shortened hormone-free interval 2, 3

Administration Guidelines

  • Slynd can be taken with or without food 1
  • Start taking Slynd on the first day of menstrual bleeding (Day 1 start) 1
  • For postpartum women who do not breastfeed or after a second-trimester abortion, start no earlier than 4 weeks postpartum due to increased risk of thromboembolism 1

Missed Dose Instructions

  • If one active tablet is missed: Take the missed tablet as soon as possible and take the next tablet at the regular time 1
  • If two consecutive active tablets are missed in Week 1 or Week 2: Take two tablets as soon as possible and two tablets the next day, then continue with one tablet daily 1
  • If two consecutive active tablets are missed in Week 3 or Week 4: Discard the current pack and start a new pack immediately 1
  • If three or more consecutive active tablets are missed: Discard the current pack and start a new pack immediately 1
  • Use backup contraception (such as condoms) for 7 days after missing two or more tablets 1

Special Considerations

  • In case of severe vomiting or diarrhea within 3-4 hours after taking a tablet, consider it a missed tablet and follow missed dose instructions 1
  • Drospirenone is contraindicated in women with renal impairment, adrenal insufficiency, or high risk of arterial or venous thrombotic diseases 1
  • Unlike traditional combined oral contraceptives, Slynd is an estrogen-free formulation, containing only drospirenone 2, 4

Clinical Benefits

  • The 4 mg dosage was specifically chosen to inhibit ovulation while maintaining plasma estradiol levels at early follicular phase levels 2
  • Drospirenone has antimineralocorticoid, antiandrogenic, and antigonadotropic properties 2, 5
  • Clinical studies have shown good efficacy with a favorable bleeding pattern and very low cardiovascular side effects 2

Monitoring

  • Monitor for common side effects including potential changes in blood pressure due to drospirenone's antimineralocorticoid activity 5
  • Be aware of potential drug interactions, particularly with medications that may increase potassium levels 1
