Duration of Symptoms in Facial Herpes Zoster
In immunocompetent patients with facial herpes zoster, symptoms typically last approximately 2 weeks, with lesions continuing to erupt for 4-6 days and gradually healing thereafter. 1, 2
Clinical Course Timeline
- Prodromal Phase: Pain, burning, or tingling often precedes the skin rash by 24-72 hours (sometimes longer) 1, 2
- Active Eruption Phase:
- Resolution Phase: Complete healing typically occurs within approximately 2 weeks in otherwise healthy individuals 1, 3
Factors Affecting Duration
Patient Factors That May Prolong Symptoms
Immunocompromised Status:
Advanced Age:
Treatment Impact on Duration
- Antiviral Therapy:
- High-dose IV acyclovir remains the treatment of choice for VZV infections in compromised hosts 1
- Oral antivirals (acyclovir, famciclovir, valacyclovir) are beneficial for VZV infections in otherwise healthy hosts 1, 6
- Antiviral medications can reduce the intensity of acute pain, accelerate healing of the vesicular rash, and reduce the duration of viral excretion 7
- Treatment is most effective when started within 72 hours after the onset of the rash 8, 4
Complications That May Extend Symptom Duration
Secondary Infection: Bacterial and fungal superinfections can complicate healing, particularly in immunocompromised patients 1, 2
Postherpetic Neuralgia (PHN):
