Ciprodex (Ciprofloxacin/Dexamethasone) Administration for Otitis Externa
For otitis externa, administer 4 drops of Ciprodex into the affected ear(s) twice daily for 7 days, even if symptoms improve sooner, to prevent relapse of infection. 1
Proper Administration Technique
- Have the patient lie down with the affected ear facing upward 1
- Instill 4 drops into the ear canal, enough to fill it 1
- Patient should remain in this position for 3-5 minutes to allow adequate penetration 1
- Perform a gentle to-and-fro movement of the ear or press with an in/out movement on the tragus to help distribute the drops 1
- After administration, have patient sit up and wipe off any excess drops 1
Treatment Duration and Expected Response
- Continue treatment for at least 7 days, even if symptoms improve sooner 1, 2
- Most patients experience significant improvement within 48-72 hours 2
- Complete resolution of symptoms typically occurs within 7 days 2
- If symptoms persist beyond 7 days, reassess the patient and consider extending treatment for up to an additional 7 days 2
Special Considerations
- If the ear canal is obstructed by debris or discharge, clean it before administering drops to ensure medication reaches the infected area 2
- For severe swelling where drops cannot penetrate, a physician may place an ear wick to facilitate medication delivery 2
- If a wick is placed, it will typically fall out on its own as inflammation decreases 1
- For patients with a known or suspected perforated tympanic membrane or tympanostomy tube, Ciprodex is appropriate as it is non-ototoxic 2, 3
Patient Instructions During Treatment
- Keep the ear dry; avoid swimming or submerging the head in water 2
- Use earplugs or cotton with petroleum jelly before showering or hair washing 1
- Avoid inserting anything into the ear canal, including cotton-tipped swabs 1
- For pain management during the first few days, oral analgesics may be needed until the drops begin working 2
- Do not use pain-relieving (anesthetic) eardrops as they can mask symptoms of delayed response to therapy 1
Monitoring and Follow-up
- If symptoms do not improve within 48-72 hours, the patient should be reassessed 2
- Complete follow-up is warranted if symptoms fail to resolve completely by 2 weeks after initiation of therapy 2
- Treatment failure may be due to obstructed ear canal, poor adherence, misdiagnosis, or unusual pathogens 2

- The combination of ciprofloxacin and dexamethasone provides both antimicrobial coverage against common pathogens (Pseudomonas aeruginosa and Staphylococcus aureus) and anti-inflammatory effects 2, 3
- Ciprodex is generally well-tolerated with minimal side effects, which may include local rash, irritation, or discomfort 1
Remember that systemic antibiotics are not recommended for uncomplicated otitis externa and should be reserved for cases with extension outside the ear canal or specific host factors requiring systemic therapy 2.
