Differential Diagnosis for Normal H&H, Normal MCV, Low RDW, Low RDW-SD, and MPV

Single Most Likely Diagnosis

  • Iron Deficiency Anemia (early stages): In the early stages of iron deficiency anemia, the red blood cell indices might not be significantly altered, and the RDW can be low due to the uniformity of the red blood cell size. The MPV (Mean Platelet Volume) can be normal or slightly decreased.

Other Likely Diagnoses

  • Anisopoikilocytosis (mild): This condition refers to the variation in size and shape of red blood cells. However, if mild, it might not significantly affect the RDW or MCV, and the MPV could remain within normal limits.
  • Thalassemia trait: Some forms of thalassemia trait can present with normal or near-normal hemoglobin and hematocrit levels, normal MCV, and low RDW due to the uniform microcytosis. The MPV is usually normal.
  • Sideroblastic anemia: This condition can have a varied presentation, including normal H&H, MCV, and low RDW, especially if it's not significantly affecting red blood cell production. The MPV can be normal.

Do Not Miss Diagnoses

  • Myeloproliferative Neoplasms (MPN): Although less likely, conditions like essential thrombocythemia or polycythemia vera can sometimes present with normal H&H and MCV. A low RDW and normal MPV do not rule out these conditions, as they can have a varied presentation.

Rare Diagnoses

  • Hereditary Spherocytosis (mild): A mild form of this condition could potentially present with near-normal indices, although typically, it leads to an increased RDW due to the variation in red blood cell size.
  • Red Cell Membrane Disorders: Certain disorders affecting the red blood cell membrane, like hereditary elliptocytosis, might have a mild presentation with normal or near-normal blood cell indices, including a low RDW and normal MPV.
