Treatment of Pearly Penile Papules
Pearly penile papules (PPP) are benign anatomical variants that require no treatment unless the patient experiences significant psychological distress, in which case ablative laser therapy—particularly CO2 or erbium:YAG laser—is the treatment of choice. 1, 2, 3
Key Clinical Distinction
PPP must be distinguished from sexually transmitted infections, particularly condyloma acuminata (genital warts), as they are completely benign and not associated with HPV or any infectious process. 2 The clinical features that differentiate PPP from genital warts include:
- Uniform, dome-shaped papules arranged in one to two orderly rows around the corona of the glans penis 2
- Size typically 1-3 mm in diameter 1
- Symmetrical distribution and consistent appearance 2
- No association with sexual activity or transmission 2
This distinction is critical because the guidelines for genital warts (which involve cryotherapy, podophyllin, TCA/BCA, imiquimod, and surgical removal) are completely inappropriate for PPP. 4
Treatment Algorithm
First-Line Management: Reassurance
Patient education and reassurance should be the initial approach for all patients with PPP. 2, 3 Emphasize that:
- These are normal anatomical variants, not a disease 2
- They are not sexually transmitted 2
- They pose no health risk 2
- No treatment is medically necessary 3
Second-Line Management: Ablative Treatment (If Patient Remains Distressed)
For patients who continue to experience psychological distress after counseling, ablative laser therapy is the preferred treatment modality. 1, 3

Cryotherapy and electrodessication with curettage (ED&C) are older treatment modalities but carry higher risks of scarring, pain, and pigmentary changes compared to laser therapy. 1, 2, 3
Important Clinical Pitfalls
Do not confuse PPP with bowenoid papulosis, which appears as raised papules on the penile shaft in younger sexually active men with HPV exposure and requires different management. 4 Bowenoid papulosis is a form of penile intraepithelial neoplasia with malignant potential, whereas PPP is entirely benign. 4
Do not biopsy PPP unless the diagnosis is uncertain—the clinical presentation is typically diagnostic, and biopsy is unnecessary and may cause scarring. 2
Cosmetic Outcome Considerations
Laser therapy provides superior cosmetic outcomes compared to traditional destructive methods (cryotherapy, ED&C) with minimal scarring, pain, and pigmentary changes. 1, 3 This is particularly important given that the sole indication for treatment is cosmetic/psychological distress. 2, 3
