Home Blood Pressure Measurement Instructions for a Study
For a blood pressure study, patients should measure BP twice daily (morning and evening) for 7 consecutive days, taking 2 readings per session 1-2 minutes apart, after 5 minutes of seated rest, and exclude the first day's measurements from analysis. 1
Equipment Requirements
- Use a validated, automated upper-arm oscillometric device with memory storage capability 1
- Ensure appropriate cuff size that encircles 80% of the arm circumference 1
- Verify device accuracy annually by comparing with office measurements 1
Pre-Measurement Preparation
Patients must adhere to these conditions before each measurement:
- Avoid caffeine, tobacco, and exercise for 30 minutes before measurement 1
- Empty bladder before measuring 1
- Rest quietly for 5 minutes in a seated position 1
- Remove all clothing covering the cuff placement area 1
Proper Measurement Technique
Body Positioning
- Sit in a chair with back supported and feet flat on the floor (not dangling) 1
- Keep legs uncrossed 2
- Support the arm on a flat surface at heart level (mid-sternum) 1
- Remain silent during the entire measurement period 1
Measurement Protocol
- Take 2 readings per session, separated by 1-2 minutes 1

Important Caveats
If the 3-day average is close to treatment threshold, continue measurements for the full 7 days to ensure reliability 1. Research shows that while 3 days provides reasonable estimates, at least 4.5 days are needed for reliable hypertension diagnosis 3.
Contraindications: This protocol may not work well in patients with atrial fibrillation or frequent ectopic beats, as oscillometric devices may produce inaccurate readings in these conditions 1. In anxious patients who become obsessed with readings, frequent monitoring should be discouraged 1.
Device maintenance: Devices older than 4 years may become inaccurate and should be replaced if found to be unreliable 1.
