Elevated homocysteine and methylmalonic acid (MMA) are sensitive functional markers of vitamin B12 deficiency, with MMA being highly specific for B12 deficiency (elevated in 98.4% of cases) while homocysteine is sensitive but less specific as it also rises with folate deficiency. 1

Metabolic B12 deficiency is defined as serum B12 below 258 pmol/L with elevated homocysteine or MMA, and is frequently missed because clinicians accept normal-range B12 levels as ruling out deficiency. 5
