Tetanus Prophylaxis Recommendation
Give a booster dose of Tdap now (Option B). For Clean, Minor Wounds

Why Tdap Over Td
Tdap is the preferred choice for persons aged ≥11 years who have not previously received Tdap or whose Tdap history is unknown when a tetanus toxoid-containing vaccine is indicated for wound management. 1, 2
The 2020 ACIP guidelines updated recommendations to allow either Td or Tdap where previously only Td was recommended, providing greater flexibility. 1 However, if the patient has never received Tdap or this is unknown, Tdap should be given to provide pertussis protection in addition to tetanus and diphtheria. 1, 2
Safety Considerations
- The interval of 12 years since the last tetanus vaccine is well beyond any safety concerns 1, 4
- Studies demonstrate that Tdap can be safely administered at intervals as short as 18 months after a previous tetanus-containing vaccine, with only minimal increases in local injection site reactions 4
- ACIP concluded in 2010 that Tdap should not be delayed when indicated and should be administered regardless of the interval since the last tetanus or diphtheria toxoid-containing vaccine 1
Common Pitfalls to Avoid
- Do not wait to give tetanus prophylaxis in wound management when >5 years have elapsed for tetanus-prone wounds or >10 years for clean wounds 1, 3
- Do not give TIG to patients with documented complete primary vaccination series (≥3 doses) unless they are severely immunocompromised or have HIV infection with a contaminated wound 1, 2
- Do not assume the patient previously received Tdap without documentation—if unknown, give Tdap rather than Td 1, 2
- Ensure proper wound care including cleaning and debridement, as vaccination is adjunctive to proper wound management 2, 5
