Can Prempro Cause Hyponatremia?
There is no direct evidence that Prempro (conjugated estrogens/medroxyprogesterone acetate) causes hyponatremia, and this is not a recognized adverse effect of this medication based on available guidelines and research.

Lack of Direct Association
- Multiple studies examining conjugated estrogens with medroxyprogesterone acetate have not identified hyponatremia as an adverse effect 1, 2
- Research evaluating metabolic effects of various CE/MPA doses (including 0.625 mg CE/2.5 mg MPA, which is the Prempro formulation) focused on lipids, coagulation factors, and carbohydrate metabolism without reporting electrolyte disturbances 1
- A 2-year randomized controlled trial of 822 women receiving various CE/MPA combinations did not report hyponatremia as an adverse event 2
Known Adverse Effects of Medroxyprogesterone
- Injectable depot medroxyprogesterone acetate (DMPA) is associated with fluid retention, which is the opposite physiologic effect from what would cause hyponatremia 3
- The primary concerns with medroxyprogesterone relate to thrombotic risk, not electrolyte abnormalities 4
- Progestins can decrease antithrombin III and protein S levels, affecting coagulation rather than sodium balance 4, 1
Contrast with Known Hyponatremia-Causing Medications

Clinical Implications
If Hyponatremia Occurs in a Patient Taking Prempro
- Investigate alternative causes including diuretics, SSRIs, carbamazepine, or syndrome of inappropriate antidiuretic hormone (SIADH) from other etiologies 3, 5, 6
- Evaluate for volume status, thyroid function, adrenal insufficiency, and renal function 3
- Consider medication reconciliation for other known hyponatremia-inducing drugs 5, 6
Important Caveat
- While Prempro is not known to cause hyponatremia, oral contraceptives containing estrogen have been associated with mild blood pressure elevation and fluid-related effects 3
- However, these effects manifest as hypertension or fluid retention, not hyponatremia 3
The absence of reported hyponatremia in multiple large-scale studies of CE/MPA combinations strongly suggests this is not a clinically significant adverse effect of Prempro.
