Absolute Contraindication: Do Not Use Nitro-BID Ointment with Tadalafil

The concomitant use of Nitro-BID (nitroglycerin) ointment with tadalafil is absolutely contraindicated and should never be done, as this combination can cause life-threatening hypotension, myocardial infarction, and death. 1, 2

Critical Timing Requirements

If a patient has taken tadalafil, at least 48 hours must elapse after the last dose before ANY form of nitrate therapy (including Nitro-BID ointment) can be safely administered. 1, 2

Tadalafil inhibits phosphodiesterase-5 (PDE5), which degrades cyclic guanosine monophosphate (cGMP); when combined with nitrates, this causes markedly exaggerated and prolonged vasodilation leading to profound hypotension. 4, 2

  • Both tadalafil and nitrates work through the nitric oxide-cGMP pathway 2
  • The synergistic effect produces severe, potentially fatal drops in blood pressure 1, 5
  • This is a Class III recommendation (harm) from the ACC/AHA, meaning the intervention should never be performed 1, 4

Emergency Situations

Even in life-threatening situations where nitrate therapy is deemed medically necessary, the 48-hour waiting period after tadalafil must be respected. 2
