GAD-7 Validation for Age Groups
The GAD-7 is validated for adults aged 18 years and older in primary care settings, and has also been validated for adolescents aged 14-18 years in general population samples. 1, 2
Adult Validation (Ages 18+)
The original GAD-7 validation was conducted in 965 primary care patients from 15 practices, establishing it as a validated diagnostic tool specifically designed for use in adult primary care settings. 1 The American Academy of Family Physicians recognizes this validation for adult populations, and the tool has been extensively used in adults across multiple clinical settings including:
- Primary care patients regardless of condition 1
- Adults with cancer 1
- Adults with cardiovascular disease 3
- Adults with epilepsy 3
- General adult populations in epidemiological studies 4
A comprehensive 2025 Cochrane review analyzed 48 studies with 19,228 participants from 27 countries, evaluating the GAD-7 in 24 different languages, confirming its diagnostic accuracy in adult populations. 3
Adolescent Validation (Ages 14-18)
The GAD-7 has been validated in adolescents aged 14-18 years with good psychometric properties comparable to those found in adults. 2 Key validation findings include:
- A large Finnish study of 111,171 adolescents aged 14-18 years demonstrated good internal consistency (Cronbach's α = 0.91) and supported the instrument's unidimensional factor structure 2
- A clinical study of 40 adolescents with GAD (mean age 14.8 ± 2.8 years) showed strong correlation with the Pediatric Anxiety Rating Scale (R = 0.65, P ≤ .001) 5
- In adolescents with GAD, scores ≥11 optimally detect moderate anxiety and scores ≥17 optimally detect severe anxiety, which differs slightly from adult cutoffs 5
Important Caveats and Limitations
The GAD-7 has NOT been validated for children under age 14. 2, 5 The available evidence only supports its use in adolescents starting at age 14 years, and clinicians should use age-appropriate anxiety measures for younger children.
The adolescent validation studies note that while psychometric properties are good, diagnostic interview studies are still needed to fully demonstrate diagnostic efficacy in the 14-18 age group. 2 The validation in adolescents is based primarily on symptom correlation and construct validity rather than diagnostic accuracy against structured clinical interviews.
Scoring Interpretation Across Ages
For adults, the recommended cutoff is:
- ≥8-10 points for detecting GAD (with ≥9 having higher positive likelihood ratio of 4.3) 1
- Severity categories: 0-4 (minimal), 5-9 (mild), 10-14 (moderate), 15-21 (severe) 1, 4
