ICD-10 Codes for Knee Edema and Pain Imaging

For a patient presenting with knee edema and pain, use ICD-10 code M25.469 (Effusion, unspecified knee) or M25.461/M25.462 (Effusion, right/left knee) to justify imaging, combined with M25.569 (Pain in unspecified knee) or M25.561/M25.562 (Pain in right/left knee). 1

Primary Diagnostic Codes

Joint Effusion Codes:

  • M25.461 - Effusion, right knee 1
  • M25.462 - Effusion, left knee 1
  • M25.469 - Effusion, unspecified knee 1

Pain Codes:

  • M25.561 - Pain in right knee 1
  • M25.562 - Pain in left knee 1
  • M25.569 - Pain in unspecified knee 1

Imaging Justification Algorithm

Initial imaging should be plain radiographs (at minimum anteroposterior and lateral views), which are justified by the above codes. 1

If radiographs are normal or show only effusion but pain persists, MRI without IV contrast is justified using the same codes, as this is the next indicated examination. 1, 2

Additional Codes to Consider Based on Clinical Context

If bone marrow edema is suspected or confirmed:

  • M87.9 - Osteonecrosis, unspecified (for bone marrow edema syndrome) 3, 4
  • M84.3 - Stress fracture (if stress-related bone marrow edema suspected) 1

If inflammatory arthritis suspected:

  • M13.869 - Other specified arthritis, unspecified knee 1

If trauma history present:

  • S83.9 - Sprain of unspecified site of knee (for acute trauma scenarios) 1

Common Pitfalls to Avoid

Do not order MRI without recent radiographs first - approximately 20% of patients with chronic knee pain inappropriately receive MRI without radiographs within the prior year. 1, 2

Consider referred pain sources - if knee radiographs are unremarkable, you may need additional codes for hip (M25.551-M25.559) or lumbar spine pathology (M54.5) to justify imaging of those regions. 1, 2

Document laterality - using specific right/left codes (M25.461/M25.462) rather than unspecified codes improves reimbursement and clinical clarity. 1

For ultrasound-guided aspiration - the effusion code (M25.46x) justifies both the ultrasound and the aspiration procedure if crystals or infection evaluation is needed. 1
