Technical Advantages of Non-Fasting Apo B Measurement

The European Society of Cardiology explicitly states that apo B assays have several technical advantages over traditional lipid measurements:

  • Unlike triglycerides which fluctuate dramatically with meals (≥20% variation), apo B levels remain relatively stable 1
  • The measurement reflects particle number rather than cholesterol or triglyceride content, making it independent of postprandial lipid fluctuations 4

Contrast with Traditional Lipid Testing

This is a critical advantage over standard lipid panels:

  • The Friedewald formula for calculating LDL-C cannot be used with non-fasting samples and becomes unreliable when triglycerides are elevated 1, 2
