Drug Interactions Between Diltiazem and Pantoprazole
The combination of diltiazem and pantoprazole is safe to use together without dose adjustment, as pantoprazole has minimal interaction potential with the cytochrome P450 system and does not affect diltiazem pharmacokinetics. 1, 2
Mechanism and Safety Profile
Pantoprazole demonstrates a uniquely low propensity for drug-drug interactions compared to other proton pump inhibitors:
- Pantoprazole has enhanced selectivity to the gastric H+/K+-ATPase and shows markedly lower affinity to cytochrome P450 enzymes compared to omeprazole and lansoprazole 2, 3
- Multiple pharmacokinetic studies confirm that pantoprazole does not affect the metabolism of drugs processed through the CYP450 system, including calcium channel blockers like nifedipine (a close structural analog to diltiazem) 1, 2
- Pantoprazole neither induces nor inhibits drug-metabolizing enzymes, as demonstrated by lack of effect on antipyrine metabolism and urinary excretion of induction markers 1
Diltiazem's Interaction Profile
While diltiazem is a moderate CYP3A4 inhibitor, this does not create clinically relevant interactions with pantoprazole:
- Diltiazem acts as both a substrate and moderate inhibitor of CYP3A4, which is relevant when combined with statins (particularly simvastatin and lovastatin) but not with pantoprazole 4
- The American Heart Association guidelines note that diltiazem's CYP3A4 interactions primarily affect drugs heavily metabolized through this pathway, which does not include pantoprazole 4
Clinical Management
No special monitoring or dose adjustments are required when combining these medications:
- Continue both medications at standard therapeutic doses without modification 1, 2
- Standard monitoring for diltiazem (blood pressure, heart rate) and pantoprazole (symptom control) remains appropriate without additional surveillance 4
Important Caveats
Be aware of diltiazem's interactions with OTHER commonly co-prescribed medications:
- Diltiazem significantly increases simvastatin exposure by 5-fold and lovastatin by 3.6-fold; limit simvastatin to ≤10 mg daily and lovastatin to ≤20 mg daily when combined with diltiazem 4
- Diltiazem has additive effects with other AV nodal-blocking agents (beta-blockers, digoxin); avoid routine combination with beta-blockers due to increased bradycardia and heart block risk 4
- Use caution combining diltiazem with drugs that prolong QT interval, though this is not a concern with pantoprazole specifically 5
Contrast pantoprazole with omeprazole, which DOES have significant interaction potential:
