Important Context: Sertraline is NOT First-Line for Uremic Pruritus

First-Line Management of Uremic Pruritus
Gabapentin is the preferred pharmacologic agent for uremic pruritus, dosed at 100-300 mg after each dialysis session (three times weekly), which is significantly more effective than other options. 5
Before any pharmacologic therapy, optimize: 5
- Dialysis adequacy (target Kt/V ~1.6)
- Calcium-phosphate balance and parathyroid hormone control
- Anemia correction with erythropoietin

When to Consider Sertraline
Use sertraline for uremic pruritus only when: 5, 1, 2
- Gabapentin has failed or is contraindicated
