Ang 78-anyos na pasyente ay may impeksyon sa ihi (UTI) na nakikita sa laboratory results (blood at leukocyte esterase sa urine, pus cells 5-10, RBC 5-10) kasama ng sintomas ng hirap at hapdi sa pag-ihi. 1, 2 Ang sakit sa tiyan, pagsusuka, mainit na dibdib at maasim na panlasa ay mga sintomas ng GERD (acid reflux). 1
Paliwanag ng Kondisyon
Ang UTI ay impeksyon ng bladder na dulot ng bacteria, karaniwan sa matatandang babae. 3 Ang GERD naman ay pagtaas ng acid mula sa tiyan papunta sa esophagus na nagdudulot ng sunog na pakiramdam at maasim na lasa. 1
Gamot at Dosage (PhilHealth Konsulta Formulary)
Para sa UTI:
- Trimethoprim-Sulfamethoxazole 160/800mg (Co-trimoxazole) - 1 tablet, dalawang beses sa isang araw, 3 araw (Total: 6 tablets) 4, 5, 6

Para sa Ubo:
- Ambroxol 30mg - 1 tablet, tatlong beses sa isang araw, 5 araw (Total: 15 tablets) 1

Para sa GERD/Sikmura:
- Omeprazole 20mg - 1 capsule, isang beses sa umaga bago kumain, 14 araw (Total: 14 capsules) 1

- Aluminum Hydroxide + Magnesium Hydroxide suspension - 1-2 tablespoon kung kinakailangan para sa mabilis na relief (Total: 1 bottle 120ml) 1

Rekomendasyon:
- Uminom ng maraming tubig (8-10 baso sa isang araw) 5

This certifies that patient has been seen and examined today with the diagnosis of: Urinary Tract Infection (UTI) with Gastroesophageal Reflux Disease (GERD) and recommendation of: Oral antibiotics for 3 days, antacid therapy for 14 days, increased fluid intake, and dietary modifications. Follow-up if symptoms persist or worsen after 3 days of treatment. This medical certificate is issued upon the request of the patient for PhilHealth documentation and treatment compliance.
