Super B Complex and Eliquis (Apixaban) Interaction

Super B complex vitamins do not have clinically significant interactions with Eliquis (apixaban) and can be safely taken together.

Pharmacokinetic Interaction Assessment

The key to understanding drug interactions with apixaban lies in its metabolic pathways:

  • Apixaban is metabolized primarily through CYP3A4 enzymes and P-glycoprotein (P-gp)/BCRP transporters 1
  • Clinically significant interactions occur only with medications that are moderate to strong inhibitors or inducers of BOTH CYP3A4 AND P-gp pathways simultaneously 1

When evaluating potential apixaban interactions, the critical factors are:

  • Strong CYP3A4 inhibitors (such as ketoconazole, clarithromycin, ritonavir) significantly increase apixaban exposure and bleeding risk 1

  • No dose adjustment of apixaban is required when adding B-complex vitamins
  • Individual patient factors such as age, renal function (CrCl), and other concomitant medications should still be assessed 2

Common Pitfalls to Avoid

  • Do not confuse vitamin K with B-complex vitamins - vitamin K affects warfarin, not apixaban 1
  • Be vigilant about OTHER medications the patient may be taking, as 55-60% of patients on anticoagulation take concomitant medications that DO increase bleeding risk 3, 4
  • Avoid combining apixaban with antiplatelet agents or NSAIDs without careful risk-benefit assessment, as these combinations substantially increase bleeding risk 1
  • Patients with severe renal impairment (CrCl <15 mL/min) or severe hepatic impairment require special consideration for apixaban dosing regardless of vitamin supplementation 1
