Alternatives to Gabapentin for pain management include pregabalin, duloxetine, venlafaxine, and capsaicin. These medications have been shown to be effective in managing neuropathic pain, particularly in patients with diabetic peripheral neuropathy (DPN) 1.

Alternatives to Gabapentin

  • Pregabalin: Eight high-quality studies and seven medium-quality studies support the role of pregabalin in treatment of pain in DPN, with a number needed to treat (NNT) of 6-7 for moderate benefit 1.
  • Duloxetine: Two high-quality studies and five medium-quality studies support the role of duloxetine in the treatment of pain in DPN, with an NNT of 6 for moderate benefit 1.
  • Venlafaxine: A high-quality study supports the role of venlafaxine in the treatment of pain in DPN, with an NNT of 3.1 for at least moderate pain relief 1.
  • Capsaicin: A recent head-to-head trial suggested therapeutic equivalency for capsaicin, with one high-quality study reported for the treatment of pain in DPN using an 8% patch 1.

Other Options

  • Tricyclic antidepressants (TCAs): Two high-quality studies and two medium-quality studies support the treatment of pain in DPN, with an NNT of 3.6 for at least moderate pain relief 1.
  • Sodium channel blockers: Five medium-quality studies support the role of sodium channel blockers in treating pain in DPN, although the evidence is not as strong as for other alternatives 1.
