First-Line Therapy
- Topical fluconazole ear drops are the preferred treatment modality for otomycosis, as they achieve high local concentrations at the site of infection while minimizing systemic exposure. 1

Important Caveat About Systemic Fluconazole
- Systemic (oral) fluconazole is NOT recommended for otomycosis because fungal isolates from otomycosis show 100% resistance to fluconazole in susceptibility testing. 2

- For yeast-predominant otomycosis (Candida species, 16.52% of cases), nystatin shows 88.24% sensitivity, followed by amphotericin B at 82.35%. 2
