Maximum Dose of Dayvigo (Lemborexant)
The maximum recommended dose of DAYVIGO is 10 mg taken no more than once per night, immediately before going to bed with at least 7 hours remaining before awakening. 1
Standard Dosing
- The recommended starting dose is 5 mg taken no more than once per night 1
- The dose may be increased to the maximum of 10 mg based on clinical response and tolerability 1
- DAYVIGO must be taken immediately before bed with at least 7 hours of remaining sleep time 1
- Taking the medication with or soon after a meal will delay sleep onset 1
Dose Reductions Required in Specific Situations
Hepatic Impairment
- Moderate hepatic impairment: Maximum dose is 5 mg once nightly 1
- Severe hepatic impairment: DAYVIGO is not recommended 1
Drug Interactions with CYP3A Inhibitors
- Strong or moderate CYP3A inhibitors: Avoid concomitant use entirely 1
- Weak CYP3A inhibitors: Maximum dose is 5 mg once nightly 1
Drug Interactions with CYP3A Inducers
- Strong or moderate CYP3A inducers: Avoid concomitant use 1
Critical Safety Considerations
CNS depression and next-day impairment are the primary safety concerns. 1
- Driving ability was impaired in subjects taking the 10 mg dose 1
- CNS depressant effects may persist for several days after discontinuation 1
- The risk of daytime impairment increases if taken with less than a full night of sleep remaining or at higher than recommended doses 1
- Elderly patients are at higher risk of falls due to drowsiness 1
Contraindications and Warnings
- Absolute contraindication: Narcolepsy 1
- Avoid alcohol consumption due to additive CNS depression 1
- Do not combine with other insomnia medications 1
- Dosage adjustments needed when co-administered with other CNS depressants (benzodiazepines, opioids, tricyclic antidepressants) 1
Unique Adverse Effects
- Sleep paralysis and hypnagogic/hypnopompic hallucinations can occur 1
- Cataplexy-like symptoms including periods of leg weakness lasting seconds to minutes may occur 1
- Complex sleep behaviors (sleep-walking, sleep-driving, eating, making phone calls) have been reported 1
Common Pitfall to Avoid
Never exceed 10 mg in any patient, regardless of inadequate response. If the maximum dose of 10 mg is ineffective after appropriate trial, consider switching to an alternative medication class rather than exceeding the maximum recommended dose, as higher doses increase the risk of next-day impairment and CNS depression without established additional benefit. 1
