How to Prescribe Epiduo (Adapalene 0.1%/Benzoyl Peroxide 2.5%)
Indication and Patient Selection
Epiduo is indicated for topical treatment of acne vulgaris in patients ≥9 years of age, with optimal use in those presenting with both comedones and inflammatory lesions. 1
- Safety and effectiveness have not been established in pediatric patients <9 years of age 1
- Most effective for patients with mild to moderate acne who have both inflammatory and noninflammatory lesions 2
- Can be used as monotherapy for mild to moderate acne or combined with oral antibiotics for severe acne 2
Dosing Instructions
Apply a thin film to affected areas of the face or trunk once daily after washing, using a pea-sized amount for each area of the face (forehead, chin, and each cheek). 1
- Apply once daily at bedtime for optimal results 3
- Cover the entire affected area, not just visible lesions—for example, if acne is on the face, apply to the entire face 1
- Do not apply more than once daily; excessive application will not provide faster results but may worsen skin irritation 4
Application Technique
- Thoroughly cleanse the skin with a gentle cleanser and pat dry before application 1
- Apply approximately 1.0 g of gel to the face per application 3
- Avoid contact with eyes, mouth, nasal creases, and mucous membranes 5
Expected Timeline for Results
- Clinically visible improvements typically occur by the third week of therapy 5
- Significant lesion reductions can be observed as early as week 1 6, 7
- Maximum lesion reduction occurs after approximately 8-12 weeks of continuous use 5

Managing Side Effects

- Weather extremes (wind, cold) may be irritating and should be avoided 1
- Use with caution when combining with other potentially irritating topical products, especially those containing sulfur, resorcinol, or salicylic acid 1
Special Populations
- Pregnancy Category C: Use only if benefits outweigh risks 1
- Nursing mothers: Unknown whether adapalene or benzoyl peroxide is excreted in human milk; use with caution 1
- Pediatric use: Not established for children <9 years of age 1
Combination Therapy Strategies
For moderate to severe acne, combine Epiduo with oral antibiotics (doxycycline or lymecycline) for 12 weeks, then continue Epiduo alone for maintenance. 2
- Epiduo plus oral doxycycline is more effective than oral doxycycline alone for severe acne 2
- After initial 12-week combination therapy, continue Epiduo monotherapy for at least 6 additional months to maintain response and achieve further improvement 2
- Do not combine with other topical antibiotics, as Epiduo already contains benzoyl peroxide which prevents bacterial resistance 9
Long-Term Management
- Continuing use is normally required to maintain satisfactory clinical response; discontinuation often leads to acne recurrence 5
- Long-term use (up to 12 months) demonstrates sustained efficacy with continued improvement and acceptable tolerability 6, 8
- Mean acne severity continues to improve from month 3 to month 9 of treatment 8
Common Pitfalls to Avoid
- Never use topical antibiotics as monotherapy—always combine with benzoyl peroxide to prevent bacterial resistance 9
- Do not increase application frequency beyond once daily, as this increases irritation without improving efficacy 4
- Do not apply to broken or eczematous skin 1
- Avoid concurrent use of other retinoids or potentially irritating agents without careful consideration 1
