Side Effects of Abilify (Aripiprazole)

Abilify causes a wide range of side effects, with the most common being nausea, vomiting, constipation, headache, dizziness, akathisia (inner restlessness), insomnia, anxiety, and blurred vision in adults, while children more frequently experience sedation, weight gain, and extrapyramidal symptoms. 1

Common Side Effects

Adults

The FDA-approved labeling identifies the following most common adverse effects: 1

Impulse Control Disorders

Unusual and compulsive urges may develop, including: 1

  • Pathological gambling
  • Compulsive eating or binge eating

Depression and Suicidality

Depression occurs in approximately 1% of patients, and appropriate counseling is recommended before initiating therapy to prevent worsening of pre-existing depression or emergence of suicidal ideation. 3

Extrapyramidal Symptoms

While aripiprazole has a lower risk of extrapyramidal symptoms compared to first-generation antipsychotics, these effects still occur: 4, 5

Common pitfall: The FDA label emphasizes that many gastrointestinal side effects occur early in treatment—70-80% within the first 2 weeks—with 75-80% being mild and 60-65% resolving within the first month. 3 This temporal pattern should guide patient counseling and management decisions.

Weight monitoring: Approximately 12% of patients experience 5-10% body weight loss, compared to 5% on placebo. 3 For patients losing more than 5% of baseline body weight, discontinuation should be considered. 3

Drug interactions: Monitor for interactions with strong CYP450 inducers (rifampin, phenobarbital, carbamazepine, phenytoin), which may decrease efficacy, and CYP2D6/3A4 inhibitors, which may increase aripiprazole levels and side effects. 2, 4
