Rybelsus Dosing Protocol
Start Rybelsus at 3 mg once daily for 1 month, then increase to 7 mg once daily for at least 1 month, with a maximum dose of 14 mg once daily if additional glycemic control is needed. 1

- Initial dose: 3 mg once daily for 1 month 1

- Many patients achieve adequate glycemic control at 7 mg and can remain on this submaximal dose long-term without further escalation 1
Critical Administration Requirements
Rybelsus must be taken on an empty stomach with no more than 4 ounces (120 mL) of plain water, at least 30 minutes before the first food, beverage, or other oral medications of the day. 1
- This administration protocol is absolutely critical for absorption and efficacy 1
- Taking Rybelsus with food, other beverages, or too close to meals significantly reduces absorption and therapeutic effect 1
- The gradual dose titration schedule is specifically designed to minimize gastrointestinal side effects 1
Renal and Hepatic Considerations
- No dose adjustment is required for any degree of renal impairment, including severe renal impairment or end-stage renal disease 1
- Semaglutide can be used without dose adjustment across all stages of chronic kidney disease 1
- No dose adjustment is needed for hepatic impairment 2
Common Side Effects and Management
- Gastrointestinal effects are the most common adverse events, including nausea, vomiting, diarrhea, abdominal pain, and constipation 1
- Most GI side effects are mild to moderate and transient, typically resolving with continued treatment 1
- The slow titration protocol (starting at 3 mg for a full month) helps minimize these gastrointestinal symptoms 1
Special Population Considerations
- Contraindicated in patients with personal or family history of medullary thyroid cancer or Multiple Endocrine Neoplasia syndrome type 2 (MEN2) 1
- For females using oral contraceptives, switch to a non-oral contraceptive method or add a barrier method during Rybelsus initiation and dose escalation 1
- Monitor for hypoglycemia when used with insulin or insulin secretagogues (sulfonylureas), as dose adjustments of these medications may be necessary 1
- For patients planning to fast during Ramadan, titrate to at least 7 mg approximately 2-3 weeks before Ramadan to ensure therapeutic efficacy and tolerance 1
