Tadalafil Should NOT Be Used for Chest Pain at Home

Tadalafil is absolutely contraindicated in patients experiencing chest pain, particularly anginal chest pain, because it can cause life-threatening hypotension if nitrates are needed for emergency treatment. 1

Critical Safety Concerns

Absolute Contraindication with Nitrates

  • Patients experiencing chest pain requiring nitroglycerin must not have taken tadalafil within the previous 48 hours, as the combination can cause severe, potentially fatal hypotension 1

  • Call emergency services if experiencing symptoms suggestive of acute coronary syndrome 5
  • Not take any medications that could interfere with emergency cardiac treatment 1

Common Pitfall to Avoid

The most dangerous misconception is that because tadalafil has cardiovascular effects in PAH, it might help with cardiac chest pain. In reality, tadalafil's vasodilatory properties can complicate emergency cardiac care and create life-threatening drug interactions with standard chest pain treatments 1, 6.
