When Fever Duration Should Raise Concern
Persistent high fever beyond 3 days warrants clinical reassessment, particularly when accompanied by:
- Severe cough, bloody sputum, or chest pain 2
- Rapid respiratory rate, dyspnea, or cyanosis 2
- Mental status changes (lethargy, confusion, restlessness) 2
- Severe vomiting leading to dehydration 2

Populations at Risk for Prolonged Fever
Certain groups may experience longer fever duration:
- Young children often have prolonged illness and can remain febrile longer than adults 1

Clinical Monitoring Recommendations
Patients should be reassessed within 2 days if they have: 1

- Age >65 years
- Relevant comorbidities
- Tachypnea or worsening dyspnea
Important caveat: While fever typically resolves in 3-4 days, cough and malaise commonly persist for more than 2 weeks even after fever has resolved, and this does not automatically indicate complications 2, 1
