Undiluted meropenem injection vials (dry powder) should be stored at controlled room temperature 20°C-25°C (68°F-77°F) and should never be refrigerated or frozen in their unreconstituted form. 1
Storage Requirements for Unreconstituted Vials
Store dry powder vials at room temperature: The FDA-approved labeling explicitly states that meropenem dry powder should be stored at controlled room temperature 20°C-25°C (68°F-77°F) per USP Controlled Room Temperature standards. 1
Do not refrigerate unreconstituted vials: Unlike many other injectable antibiotics that require refrigeration, meropenem powder vials are stable at room temperature and do not need cold storage prior to reconstitution. 1
Protect from excessive heat and light: While the primary storage requirement is room temperature, avoid exposure to direct heat sources or prolonged direct sunlight, as these can degrade the antibiotic powder. 2
Critical Storage Distinctions
Important caveat: The room temperature storage applies ONLY to unreconstituted (dry powder) vials. Once reconstituted, meropenem solutions have dramatically different and much more restrictive storage requirements:
Reconstituted solutions for bolus administration: May be stored for up to 3 hours at up to 25°C (77°F) or for 13 hours at up to 5°C (41°F). 1
Reconstituted solutions for infusion in 0.9% sodium chloride: May be stored for 1 hour at up to 25°C (77°F) or 15 hours at up to 5°C (41°F). 1
Reconstituted solutions in 5% dextrose: Should be used immediately and cannot be stored. 1
Storage Monitoring and Handling
Check expiration dates: Always verify the expiration date on unreconstituted vials before use, as expired meropenem should be discarded. 2
Visual inspection: Before reconstitution, inspect vials for any discoloration, particulate matter, or damage to the vial integrity. 1

Common Pitfalls to Avoid
Do not confuse storage requirements: The most common error is applying the refrigeration requirements of reconstituted solutions to unreconstituted vials, or vice versa. Unreconstituted powder stays at room temperature; reconstituted solutions require refrigeration for extended storage. 1
Do not freeze: Freezing is never appropriate for meropenem in any form—neither the dry powder nor reconstituted solutions should be frozen. 1
