Augmentin Injection Dosing

Standard Adult Intravenous Dosing

For adults and children over 12 years, the recommended dose of Augmentin injection is 1.2 g (1000 mg amoxicillin/200 mg clavulanate) intravenously every 8 hours for most serious infections. 1

Primary Indications for IV Augmentin

  • Severe skin and soft tissue infections: 1.2 g IV every 8 hours 2
  • Complicated intra-abdominal infections: 1.2 g IV every 8 hours for 4-7 days with adequate source control 2
  • Severe respiratory infections: 1.2 g IV every 8 hours 2
  • Prophylaxis in abdominal surgery: 1.2 g IV at induction of anesthesia, followed by 600 mg IV at 8 and 16 hours for short-term prophylaxis 3

Pediatric Intravenous Dosing (>3 months)

  • Standard dosing: 200 mg/kg/day of the amoxicillin component divided every 6 hours for complicated intra-abdominal infections 4
  • Maximum daily dose: Should not exceed 4000 mg of amoxicillin per day 2, 4

Renal Impairment Adjustments

Dose reduction is mandatory for patients with severe renal impairment to prevent toxicity. 1

  • GFR 10-30 mL/min: 500 mg or 250 mg every 12 hours (severity-dependent) 1
  • GFR <10 mL/min: 500 mg or 250 mg every 24 hours (severity-dependent) 1
  • Hemodialysis patients: 500 mg or 250 mg every 24 hours, with an additional dose during and at the end of dialysis 1
  • Patients with GFR <30 mL/min should NOT receive the 875 mg tablet formulation 1

Critical Administration Guidelines

  • Infusion time: Administer over 15-30 minutes for optimal tolerance 5
  • Treatment duration: Continue for minimum 48-72 hours beyond clinical improvement or bacterial eradication 1
  • Streptococcal infections: Require minimum 10 days of therapy to prevent acute rheumatic fever 1

Common Pitfalls to Avoid

  • Do not substitute two 250 mg tablets for one 500 mg tablet - this results in excessive clavulanate dosing and increased gastrointestinal side effects 2
  • Severely impaired renal patients (GFR <30 mL/min) should never receive the 875 mg formulation due to accumulation risk 1
  • Neonates and infants ≤12 weeks require reduced dosing (maximum 30 mg/kg/day divided every 12 hours) due to immature renal function 1
