Switching from Prozac (fluoxetine) to Cymbalta (duloxetine) should be considered if the patient does not have an adequate response to fluoxetine or experiences intolerable side effects.
Considerations for Switching
- Efficacy: There is no significant difference in efficacy between fluoxetine and duloxetine for treating major depressive disorder, as shown in head-to-head RCTs of good to fair quality 1.
- Adverse Effects: Duloxetine may have a different adverse effect profile compared to fluoxetine, with a higher risk of nausea, diarrhea, and dizziness, but a lower risk of sexual dysfunction 1.
- Dose and Titration: When switching from fluoxetine to duloxetine, the dose of duloxetine should be titrated gradually to minimize the risk of adverse effects, starting with a low dose of 20-30 mg/day and increasing to 60 mg/day as needed and tolerated.
- Monitoring: Patients should be closely monitored for suicidal thoughts and behaviors, as well as emergence of agitation, irritability, or unusual changes in behavior, especially during the first 1-2 months of treatment with duloxetine 1.
- Patient Preferences: The decision to switch from fluoxetine to duloxetine should take into account patient preferences, cost, and adverse effect profiles, as recommended by the American College of Physicians 1.

2.9 Switching a Patient to or from a Monoamine Oxidase Inhibitor (MAOI) Intended to Treat Psychiatric Disorders At least 14 days should elapse between discontinuation of an MAOI intended to treat psychiatric disorders and initiation of therapy with duloxetine delayed-release capsules Conversely, at least 5 days should be allowed after stopping duloxetine delayed-release capsules before starting an MAOI intended to treat psychiatric disorders
There are no specific considerations mentioned in the label for switching from Prozac (fluoxetine) to Cymbalta (duloxetine). However, since both are antidepressants, caution should be exercised when switching.
