Doxycycline Dosing for Acne
For moderate to severe inflammatory acne, doxycycline should be dosed at 200 mg on day 1 (given as 100 mg every 12 hours), followed by 100 mg daily as maintenance, and must always be combined with topical benzoyl peroxide and a retinoid to prevent antibiotic resistance. 1, 2, 3
Standard Dosing Regimen
Adults and Children Over 100 Pounds
- Loading dose: 200 mg on day 1, administered as 100 mg every 12 hours 2, 3
- Maintenance dose: 100 mg daily thereafter 2, 3
- For severe infections, 100 mg every 12 hours may be used 3
Children 8 Years and Older Under 100 Pounds
- Loading dose: 2 mg/lb body weight divided into 2 doses on day 1 2, 3
- Maintenance dose: 1 mg/lb body weight daily (as single dose or divided into 2 doses) 2, 3
- For more severe infections, up to 2 mg/lb body weight may be used 3
Treatment Duration
Limit doxycycline treatment to 3-4 months maximum, with mandatory re-evaluation at that point to minimize bacterial resistance development. 2, 4

- After discontinuing doxycycline, continue topical benzoyl peroxide and retinoid indefinitely for maintenance 2
Mandatory Concomitant Therapy
Doxycycline monotherapy is explicitly contraindicated—you must combine it with topical benzoyl peroxide and/or a retinoid throughout treatment. 1, 2, 4
- The American Academy of Dermatology states that systemic antibiotics must be used concomitantly with benzoyl peroxide and other topical therapy 1
- This combination prevents development of antibiotic resistance and follows antibiotic stewardship principles 2, 4
- Continue topical therapy for maintenance after completing systemic antibiotics 2, 4
Alternative Dosing: Subantimicrobial Approach
For patients where standard dosing poses concerns, subantimicrobial-dose doxycycline (20 mg twice daily or 40 mg daily) has demonstrated efficacy in moderate inflammatory acne with reduced risk of antibiotic resistance. 2, 4, 5, 6
- This regimen showed 84% reduction in papules and 90% reduction in pustules at 3 months 5
- No detectable effect on antibiotic resistance of skin flora was observed 6
- However, this is considerably more expensive than generic doxycycline 100 mg 7
Absolute Contraindications
- Children under 8 years of age (risk of permanent tooth discoloration and bone growth inhibition) 2, 4, 3
- Pregnancy (Category D) 1, 4
- Nursing mothers 1
- Hypersensitivity to tetracyclines 1, 4
Critical Drug Interactions
Doxycycline interacts with multiple medications that can reduce absorption or increase adverse effects: 2, 4
- Antacids containing aluminum, calcium, or magnesium 1, 4

- Bismuth subsalicylate 2
- Proton-pump inhibitors 2

Common Adverse Effects and Management
Doxycycline causes gastrointestinal disturbances more frequently than minocycline, particularly at higher doses. 2, 4
- Photosensitivity is common—counsel patients on strict sun protection 2, 4
- If gastric irritation occurs, administer with food or milk (absorption not significantly affected) 3
- Administer with adequate fluid to reduce risk of esophageal irritation and ulceration 3
When Doxycycline Is NOT Appropriate
Do not use doxycycline for mild acne—topical therapies (retinoids, benzoyl peroxide) are first-line treatment. 2
- Reserve doxycycline for moderate to severe inflammatory acne or cases resistant to topical treatments 2, 4
- For severe acne failing oral antibiotics, consider isotretinoin 2, 4
- For women with hormonal acne components, consider combined oral contraceptives or spironolactone as adjunctive therapy 1, 4

- Doxycycline demonstrates superiority over azithromycin in randomized controlled trials 4
- Standard-dose doxycycline and minocycline show similar efficacy with no clear superiority 2
- Therapeutic antibacterial serum activity persists for 24 hours following recommended dosage 3
